Program Nurse

Pay Rate: $55 - $65 per hour

Pathways to Wellness Medication Clinic is an outpatient mental health treatment program established in 2000 to address the significant needs within our mental health community. We provide vital services for adults and youth requiring mental health management, staffed by Board Certified Psychiatrists, Nurse Practitioners, Physician Assistants, Clinical Pharmacists, Psychiatric Nurses, and Licensed Clinicians. Our mission is to offer significant relief from the symptoms of mental illness, facilitate our patients' ability to remain in the community, and avoid higher levels of care. For over 25 years, we have been delivering specialty mental health services to individuals in Alameda County.

Position Overview

We are seeking a full-time Psychiatric Registered Nurse to join our outpatient mental health clinic in Oakland. The Program Nurse will work in a high-volume, fast-paced outpatient medication clinic setting as a member of an interdisciplinary team, providing care for clients with serious mental illness and co-occurring disorders.

Key Responsibilities

• Patient Care:
• Conduct psychiatric assessments, including vital signs, treatment plans, and safety plans.
• Administer injections and coordinate laboratory tests.
• Complete medication authorizations and run CURES reports.
• Clinical Coordination:
• Provide ongoing communication and collaboration with onsite Medical Providers to ensure well-coordinated care within the program.
• Engage with acute hospitals and community resources to coordinate clients' clinical care and recovery services.
• Case Management:
• Deliver clinical case management services, ensuring comprehensive care for clients.
• Maintain accurate and timely documentation of all nursing tasks and patient interactions.

Qualifications

• Valid California license as a Registered Nurse.
• Minimum of 3 years' experience working in inpatient or outpatient mental health settings.
• Master's degree in Psychiatric Nursing is preferred but not required.
• BSN Certification as a Psychiatric/Mental Health Nurse is preferred but not required.
• Ability to thrive in a fast-paced, high-volume work environment.

Work Schedule

• Full-Time: 40 hours per week.
• Clinic Hours: Monday - Friday, 8:30 am - 5:00 pm.
• Overtime may be required.
